Job Summary Responsible for assistance with program development, implementatio! n, and successful outcomes in support of company strategies for the Patient Centered Medical Home (PCMH) care model. Continuous identification of potential candidates for case management an implementing proven evidence based strategies and interventions prioritized by the members with the highest impact potential. The Nurse Case Manager (CM) coordinates and manages care of individual members to meet multiple service needs, ensure continuity of services, and overcome fragmentation of care and services. These services provided require building, implementation, and documentation of individualized care plans and outcomes within the Case Management Module. The CM will incorporate an interdisciplinary approach to episodic management which includes the medical director, department manager and nurse reviewer. Close measurement of the return on investment (ROI) for the CM services provided. The goal of the nurse case manager is to help our members receive high quality of care and le! ngthen their time of wellness while providing cost savings to ! the health plan as well as the member. Job Requirements Education Required: Associates Degree in a Related Field Preferred: Bachelors in a Related Field
Experience 3-5 Years Related Experience
Skills Excellent verbal and written communication skills. Able to work independently and collaboratively in teams. Highly motivated, assertive, strong critical thinking skills, ability to multi-task, and a self starter. Experience with Microsoft Office Programs. Excellent typing and medical research skills Strong phone skills
Licensure/Certification/Registration MO Registered Nurse License Current CCM or must obtain within 2 years of hire
