Must have excellent knowledge of geriatric Med/Surg and Hospice Nursing and the effects that the disease and aging process has on residents and their families. Must be able to work with the Interdisciplinary Health Care Team to develop and maintain a therapeutic milieu. Directs, through charge nurses, activities of nursing staff by performing the following duties. Essential Duties and Responsibilities ADMINISTRATIVE Assess and manage the functions of the nursing staff in accordance with current rules, regulations and guidelines that govern the long-term care facility.

Participate in maintaining our written policies and procedures that govern the day-to-day functions of the Nursing Department.

Ensure that the Nursing Procedures Manual is followed by nursing personnel, and that it reflects the day-to-day nursing care procedure used by this facility.

Assist in developing methods for coordinating nursing services with other resident services to ensure the continuity of the residents total regimen of care.

Perform administrative duties such as completing medical forms, reports, evaluations, studies, charting, etc., as necessary.

Participate in the maintenance of the facilities quality assurance program for the Nursing Department.

Participate in survey (inspection) made by authorized government agencies.

Make written and oral reports/recommendations to the Director as required concerning the operation of the Nursing Department.

Meet with your assigned nursing staff at beginning of each shift to give a resident report, and give nursing assistants and medical technicians their assignments.

Be familiar with the departments policies and procedures.

Admit, transfer, and discharge residents as required, with paperwork.

Complete accident/incident reports as necessary.

Write resident charge slips and forward to the Business Office.

Ensure that the written policies and procedures that govern the day-to-day functions of the Nursing Department are followed by all nursing personnel.

Ensure all nursing personnel are following their respective job description.

Meet with the nursing staff, as well as support personnel, in planning the department’s services, programs and activities.

Interpret the department’s policies and procedures to personnel, residents, visitors and government agencies as required.

Recommend to the DON the equipment and supplies you need for the unit.

COMMITTEE FUNCTIONS Attend various committee meetings for the facility as required.

Assist in preparing written and/or oral reports of the nursing service programs an activities to submit to such committees as directed.

Implement recommendations from established committees as instructed by the department Director.

PERSONNEL FUNCTIONS Ensure that a sufficient number of licensed practical and/or registered nurses are available for all tours of duty to ensure that quality care is maintained.

Ensure that a sufficient number of nursing assistants are available for all tours of duty to ensure that routine nursing care is provided to meet the daily nursing care needs of each resident.

Develop work assignments and/or assist charge nurses in completing and performing such tasks.

Delegate to each charge nurse the administrative authority, responsibility and accountability necessary to perform their assigned duties.

Make daily rounds of the Nursing Services Department to ensure that all nursing service personnel are performing their work assignments in accordance with acceptable nursing standards. Reports findings to the Director.

Monitor absenteeism to ensure that an adequate number of nursing care personnel are on duty at all times. Follow-up call in slips are to be given to Nursing Office Manager. Report problem areas to the Director.

Review complaints and grievances made or filed by department personnel. Make appropriate reports to the Director as required.

Ensure that disciplinary action is administered fairly and without regard to race, color, creed, national origin, age, sex, religion, handicap or marital status.

Develop and maintain a good working relationship with inter department personnel, as well as other departments within the facility to assure that nursing services and activities can be adequately maintained to meet the needs of the residents.

Assure that department personnel, residents and visitors follow the departments established policies and procedures at all times.

Create and maintain an atmosphere of warmth, personal interest and positive emphasis, as well as a calm environment throughout the unit and shift.

Meet with your shift on a regular basis to assist in identifying and correcting problem areas, and/or the improvement of services.

Assess and evaluate heath issues of employees that arise during work hours.

Review and evaluate your department’s work force and make recommendations to the Director.

Participate in determining shifts staffing requirements, employee introductory and annual performance evaluation process and making recommendations to the Director concerning employee transfers, and etc.

NURSING CARE Inform nursing personnel of new admissions, their expected time of arrival, room assignment.

Ensure that rooms are ready for new admissions.

Meet newly admitted residents.

Review nurses’ notes to ensure that they are informative and descriptive of the nursing care being provided and that reflect the resident’s response to the care. Make rounds with physicians as necessary.

Encourage attending physicians to record and sign progress notes, physicians’ orders, etc., on a timely basis and in accordance with current regulations.

Make daily resident visits to observe and evaluate the residents physical and emotional status.

Monitor medication passes and treatment schedules to assure that medications are being administered as ordered and that treatments are provided as scheduled. Supervises and monitors control medications.

Review medication bubble cards and/or any other type of medication for completeness of information, accuracy in the transcription of physician orders, and adherence to stop order policies.

Provide direct nursing care as necessary.

Report problem areas to the Director. Assist in developing and implementing corrective action.

Keep the Director informed of the status of residents and other related matters through written/oral reports.

Ensure that our written procedures for the administration, storage and control of medications and supplies are being followed by all RNs and LPNs on your shift.

Meet with residents, and/or family members as necessary. Report problem areas to the Director.

Inspect the nursing service treatment areas daily to ensure that they are maintained in a clean and safe manner.

Consult with the resident’s physician in planning resident care, treatment, rehabilitation, etc., as necessary.

Assist charge nurse and/or notify the resident’s attending physician, chaplain and responsible party when there is a change in the resident’s condition.

Work with the facility’s consultants and implement those changes authorized by the Director.

Call funeral homes when requested by the family. Authorize the use of restraints when necessary and in accordance with our facilities policies and procedures.

Make independent decisions concerning nursing care. Answer emergency calls on campus for resident’s and employees. Assess, evaluate and make decisions as necessary.

Start IV’s obtain sputum, urine and other lab tests as ordered.

Take TPRs, blood pressures, etc., as necessary.

Admit, transfer and discharge residents as necessary.

Assist the charge nurse in monitoring seriously ill residents.

Inform family members of the death of resident as necessary.

STAFF DEVELOPMENT Participate in planning, conducting and scheduling of in-service training classes that provides instructions on “how to do the job”, and ensures as well educated nursing services department.

Develop, implement and maintain an effective orientation program that orients the new employee to your shift, its policies and procedures, and to his/her job duties.

Provide leadership training that includes the administrative and supervisory principles essential for your charge nurses.

Encourage your charge nurses to attend and participate in outside training programs. Schedule times as appropriate.

Assist support services in developing, implementing, and conducting in-service training programs that relate to the Nursing Services Department.

Attend and participate in continuing educational programs designed to keep you abreast of changes in your profession, as well as to maintain your license.

SAFETY AND SANITATION Monitor your shifts personnel to assure that they are following established safety regulations in the use of equipment and supplies.

Ensure that your units resident care rooms, treatment areas, etc., are maintained in a clean, safe, and sanitary manner.

Ensure that your nursing employees follow established departmental policies and procedures, including dress codes.

Ensure that your assigned personnel participate in and conduct all fire safety and disaster preparedness drills in a safe and professional manner.

Participate in the maintenance of the infection control program for monitoring communicable and/or infectious diseases among the residents and personnel.

Participate in the maintenance of the procedures for reporting hazardous conditions of equipment.

Utilize body substance precautions and other infection control guidelines.

Ensure that nursing service employees follow established hand washing techniques.

Direct personnel in response to emergency such as fire, power failure, natural disasters in absence o administrator and/or DON.

EQUIPMENT AND SUPPLY FUNCTIONS Recommend to the Director the equipment and supply needs of the department.

Assure that a stock level of medications, medical supplies, equipment and etc, is maintained on premises at all times to adequately meet the needs of the resident. Including emergency supplies and medication.

Assist the Maintenance Director in developing a preventative maintenance program for maintaining the Nursing Services Department’s equipment and supplies in a safe and operable condition.

Ensure that only trained and authorized personnel operate the department’s equipment.

Ensure that all personnel operate nursing service equipment in a safe manner. Monitor nursing procedures to ensure that nursing service supplies are used in an efficient manner to avoid waste.

CARE PLAN ASSESSMENT Develop preliminary and comprehensive assessments of the nursing needs of each resident.

In so far as practical, encourage the resident to participate in the development and review of his/her plan of care. Encourage the staff to attend care plan meetings.

Ensure that your assigned nursing assistants are aware of the resident care plans and that they are used in administering daily care to the resident.

Review nurses’ notes to determine if the care plan is being followed.

Develop and maintain a good rapport with all services involved with the care plan to ensure that a team effort is achieved in developing a comprehensive plan of care.

BUDGET AND PLANNING FUNCTIONS Keep abreast of economic conditions/situations, and recommend to the Director adjustments in nursing services that ensure the continued ability to provide quality care.

Recommend to the Director the equipment needs of the Nursing Services Department.

Knowledge, Skills and Abilities To perform this position successfully, an individual must be able to perform each essential duty satisfactorily. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions of this job.

Graduate from a accredited school of nursing. Current RN license in state of practice. Fourth year college or university program certificate; or two to four years related experience and/or training; or equivalent combination of education and experience. Must be CPR certified.

Must be knowledgeable of nursing and medical practices and procedures, as well as laws, regulations, and guidelines that pertain to long-term care. Must posses the ability to plan, organize, develop, implement, and interpret the programs, goals, objectives, policies and procedures, etc., that are necessary for providing quality care. Must have patience, tact, flexibility, a cheerful disposition and enthusiasm, as well as the willingness to handle difficult residents.
