  • As a Quality Initiatives Manager, the intent is to work together with health care entities and provide advice and guidance on potential quality improvement solutions that, if implemented by the health care entity, may lead to better patient outcomes and improved healthcare delivery surrounding glycemic control.
  • You will provide advice, expertise and solutions in the area of glycemic control to health care entities in the management of their quality improvement projects that is intended to lead to better patient outcomes and improved healthcare delivery.
  • Advise cross-functional teams (internal and external) in the creation and implementation of quality initiatives to help improve patient care at health care en! tities
  • Effectively utilize all appropriate knowledge ! (CPHQ, Six Sigma Greenbelt/Blackbelt) and resources to help support and accelerate health care entity projects around glycemic control.
  • Establish relationships across the C-Suite and D-Suite (Chief Executive Officer, Chief Operating Officer, Chief Financial Officer, Chief Medical Officer, Director of Quality and Director of Case Management etc) to determine health care entity needs for improvement in glycemic control and/or patient outcomes.
  • Utilize well defined performance metrics to support health care entities in delivering measurable improvement in quality and patient safety outcomes (e.g. core measures, readmissions, length of stay, Glycemic Control, etc.).
  • Take a solution oriented approach to addressing common QIM problems/needs/opportunities for the purpose of advancing team skill and knowledge.
  • Maintain professional and technical knowledge through established Quality Improvement networks, participation in professional societies, a! nd alignment with national quality organizations (e.g. SHM, NTOCC, Joint Commission), while offering practiced understanding of the impact standardized evidence-based practices can have on patient populations.
Requirements:
Education/Training Requirements:
  • BA/BS from an accredited school required. Focus in business or life science preferred. MBA, MHA or MPH â€" preferred.
o Certified Professional in Healthcare Quality (CPHQ) certification preferred
o Six Sigma Green/Black Belt and/or LEAN certification preferred
  • 3+ years of successful experience in the health care environment with strong background in account related work or 3+ years of extensive health care related work (case management, discharge planning, pharmacy administration, nursing, director of quality, etc).
  • Broad knowledge of principles and methods in a recognized professional field, and strong knowledge of Diabetes is preferred
  • Working knowledge of CMS Policies/Core Measures, Get with the Guidel! ines, National Patient Safety Goals, Transitions of Care/Readmissions
  • Internal candidates must have at least 18 months tenure with Sanofi and a minimum of 12 months in current position.
