Under the direction of the office Medical Director, makes advanced assessments of patients and consultation !

for treatment plans and symptom management.

ESSENTIAL FUNCTIONS

Participate in the QAPI hospice performance improvement program.

Implement the individualized plan of care and recommend revisions to the plan of care as necessary; including managing discomfort and providing symptom relief, providing specialized nursing skills related to palliative and end-of-life care, and consulting with and educating the patient and family regarding the disease process, self-care techniques, end-of-life care, and the processes for dealing with issues of ethical concern.

Inform the physician and other personnel of changes in the patient's needs and outcomes of intervention.

Provide staff/client education as needed.

Follow provisions of care according to position, as assigned by the patient’s Plan of Care. Document care in clinical notes.

Provide regular communication with all team members and those involved in the pati! ent’s care. Work in a team approach with IDT/IDG.

A! ttend all required In-service programs.

Practice exemplary infection control measures in the conduct of all job tasks.

Function as a resource for staff.

Assist Hospice Nurse and staff with wound care.

Perform patient visits as requested.

Perform Medical Assessments in the following areas: wounds, 180 day stays, crisis needs, others as requested.

Participate in On Call.

Assists with and perform other duties as assigned.

Participate in required in-services.

Required Experience
Education : Master’s Degree, Advanced Registered Nurse Practitioner experience. Family Nurse Practitioner or Geriatric Nurse Practitioner specialization.

Experience : Three (3) to five (5) years practice as ARNP to include the management of chronic medical conditions, acute exacerbations, and new acute problems of adults.

Certificates & Licenses

Current valid ARNP license, with no restrictions, in the state work will be performed. Current DEA license.

Family or Geriatric Nurse Practitioner Certification.
