The Neighborhood Nurse Coordinator is responsible for the planning, implementation, and evaluation of resident centered care planning for the r! espective neighborhood to comply with Community Quality standards, State and Regulatory Compliance. Responsibilities include the development of all nursing service personnel, performance management, evaluation, monitoring staffing needs, policy and procedures, and communication with resident and family members. Responsibilities
1. Plan, develop, organize, implement and evaluate Nursing Services program in the Care Center to ensure continuity of residentsâ total regiment of care, in accordance with current rules, regulations, and guidelines. Participate in pre-admission assessment data collection. Manage admissions, transfers, and discharge residents as required.
2. Support and evaluate and implement written policies and procedures that govern the day-to-day functions of the Care Center.
3. Responsible for a! ll aspects of care planning and documentation for respective n! eighborhood MDS.
4. In tandem with the Director of Nursing, maintains accurate staffing levels to meet Resident care needs, including monitoring of over time.
5. Monitor, evaluate, and recommend process improvements to meet Community quality, safety and risk management guidelines. Ensure compliance of reporting processes.
6. Monitor and evaluate staff development needs including orientation, performance management, and leading of self-directed work teams. Empower staff members to participate and promote Culture Change.
7. Responsible for safe medication practices including compliance with Community standards, state and federal regulations. Provide regular monitoring of nursing practice.
8. Promote an environment that meets safety and infection control guidelines and regulations including proper hand-washing techniques, resident proper transfer, proper lifting techniques, blood borne pathogens, protective equipment and fire and em! ergency standards.
9. Participate in the maintenance of the procedures for equipment and reporting hazardous conditions of equipment.
10. Ensure that all nursing service personnel are knowledgeable of the resident rights and responsibilities including CONFIDENTIALITY of all pertinent resident and employee information to assure their privacy are protected.
Background and Qualifications
1. Current RN license in state of practice
2. Graduate from an accredited school of nursing.
3. Must be CPR certified.
4. One year experience in Medicare in a Nursing home environment preferred.
Must posses the ability to make independent decisions when circumstances warrant such action. Must be knowledgeable and skilled in nursing medical practices and procedures with emphasis in the geriatric population. Must be familiar with current laws, regulations, and guidelines that pertain to long-term care. Must posses the ability to plan, organize,! develop, implement, and interpret the programs, goals, objectives, pol! icies and procedures, etc., that are necessary for providing quality care and personal growth. Must maintain a high level of professionalism and a cheerful disposition and willingness to handle difficult situations.
