Health Outreach Specialist I
Provides initial contact between Disease Management Program and specific identified members. Under direct supervision, conducts outbound motivational calls to eligible members to provide information regarding program benefits.
Primary duty is to:
Provide outbound outreach to members to close the gaps in care.
Other duties include, but are not limited to:
Conducts disease management survey via telephone and gathers appropriate member information.
Persuades members to participate in the Disease Management Program by providing information about the p! rogram, outlining member benefits and explaining available ser! vices.
Handles inbound calls as needed.
Records call details and adjusts call scripts as needed based on client requirements.
Directs member inquiries to appropriate Health Coach or Nurse Health Coaches.
Job Qualifications
Requirements:
High school diploma or the equivalent.
Six months experience in a telesales or phone role demonstrating ability to influence customers/members; or any combination of education/experience, which would provide an equivalent background.
Computer proficiency and the ability to follow defined policies and protocols required.
Knowledge of company products, services and benefits preferred.
This is a full-time, 34 hour per week position and all candidates must be available to work a shift between 7:30am and 10pm M.T.W.R.F with every other Saturday 10:30am-6pm with a day off during the week.
