Required Qualifications:
A high school diploma or an equivalent combination of education and experience from which comparable knowledge, skills and abilities can be acquired is necessary.
Ability to read, write and follow instructions is necessary.
Six months experience involving the direct care of patients in a sub-acute, acute, mental health, or residential setting, or a combination of class training and experience to equal six months experience, or one year of education beyond high school that included coursework in nursing, psychology, social work or related area is necessary.
Current certification in Basic Cardiac Life Support or will obtain within three months of hire. Preferred Qualifications:
Detailed Description:
Report and record observable changes in resident behavior and physical condition.
Assists with, or participates in re-motivation, rehabilitatio! n, re-socialization, and behavior modification programs.
Interact with patients as a part of the treatment plan, including social, recreational and treatment activities in order to provide continuing treatment and rehabilitation.
Teach and/or assist residents in self-help skills and social behavior based on the level of maturation of each resident.
Initiate and participate in group and/or individual activities with residents; escorts residents as assigned.
Participate in specialized programs such as adjunctive therapies and individual interaction with residents.
Retrieve and return meal carts and observe meal times for safety.
Plans and/or assist in planning, initiates, and participates in special programs for residents such as self-help skills and social behavior, remotivation, rehabilitation, and resocialization.
Respond to, and participate in, crisis interventions (assaultive/resistive behavior, suicide attempts, medical emergencies, etc.) in order to assure safety of p! atients, staff and the general public. Ability to perform such techniques as C.P.R and aggression management in such situations.
Perform basic nursing care and provide general daily care of residents.
Assist the physician and/or nursing personnel by: preparing residents for physical examinations, and oxygen therapy; and observing and caring for residents during treatments.
Perform and record bedside nursing activities including taking and recording patients' vital signs and collecting urine samples, etc. Deliver specimens to designated areas.
Maintain a safe and clean patient environment by keeping the area free from material which could pose a hazard to patients.
Perform grooming and hygiene (including dressing, bathing, toileting and feeding) for patients unable to do so.
Teach patients living skills (hygiene, grooming, social interaction, etc.) as a part of the treatment plan.
Provide frequent offers o! f assistance and comfort measures to patients to assure patient satisfa! ction and care needs are met.
Maintain adequate supplies and equipment.
Prepare and maintain records and reports.
Escort patients to and from medical appointments and testing procedures at other locations in the hospital.
Accompany patients on transports to other programs/facilities throughout the state.
Other duties as assigned.
