We are currently seeking! compassionate and dedicated LPNs to provide direct care to clients in the comfort of their own home. Patient care includes, but is not limited to, administering medication, g-tube, trach and vent care. We'll give you the knowledge, skill, and training you need to care for your patient.
We currently have openings in St. Louis County, St. Charles, Franklin, Jefferson, Lincoln, and St. Louis City counties.
Position Summary
Provides direct patient care and related support to medically fragile clients and their families under the supervision of a Registered Professional Nurse, and within the realm of accepted standards of practice for Licensed Practical Nurse as stated in the Nurse Practice Act for the State of Missouri.
Duties and Responsibilities Provides assistance in the development and implementation of the plan of care Performs basic patient care and treatments including but not limited to, taking blood pressure readings, wound care, measure vital signs, c! ollection of blood samples, or administration of prescribed me! dications, in accordance with the physician's treatment plan, individual plan of care, or as authorized by the client's payer's source Observes, documents and timely reports any changes in the client's condition to the appropriate personnel and/or physician Provides ongoing instruction and education of client's health care plan to family and or other health team personnel Follows established departmental policies, procedures, and objectives Practices safety, environmental, and/or infection control measures May supervise In-home Aides for Advanced Personal Care (APC) services Performs miscellaneous job-related duties as assigned by Supervisor
Minimum Job Requirements
Reliable transportation and working telephone as required by the nature of the position
Education/Licensure: Graduation from an approved School of Practical/Vocational Nursing and current Missouri LPN license or compact PN
Experience: Six (6) months experience as a practical/vocatio! nal nurse in a hospital, nursing home or home care setting within the last three (3) years is preferred.
Knowledge, Skills & Abilities
Knowledge of patient evaluation and triage procedures. Ability to provide care to patients at remote locations and demonstrate good nursing judgment with minimum supervision. Ability to deal effectively and courteously with clients, families, and co-workers. Ability to interact and exhibit an empathetic attitude toward the sick and elderly. Ability to communicate effectively, verbally and in writing.
Conditions of Employment
Successful candidate must have a satisfactory post-offer Medical Review and a "clear pre-employment background check.
May be required to submit to annual TB screenings and/Hep vaccinations
Must have proof of Auto Insurance (minimum required by law) and valid Missouri driver's license.
