The duty station is at/near the
John Cochran Medical Center; however, the
duty station may be changed to any of our medical center locations based on the
needs of the medical center. This position is in the bargaining unit. This

FUNCTIONS:
Functions
as a core member of an interdisciplinary team.
2.
Provides clinical leadership
necessary to deliver home-based medical care to a psycho-socially complex
population of chronically ill or severely disabled veterans within a defined radius
of the John Cochran Medical Center.
3.
Monitors Transitional Care Team
(TCT) consults to information is processed appropriately.
4.
Plans, implements, coordinates, and
delivers disease focused medical care as an interdisciplinary team member team
to assigned patients for up to six weeks post-discharge.
5.
Implements plan of care to reduce
patie! ntÂ's risk for re-admission.
6.
Responsible for assessment! ,
planning, coordinated provision, evaluation of individual treatment plans as
part of an interdisciplinary team for patients receiving care from the TCT
program.
7.
Identifies the patientÂ's medical
problems and defines the medical management determining the need for subspecialty
clinic and community services.
8.
Supports and directs the provision
of care by others to the TCT population.
9.
Plans, implements and coordinates patientÂ's
transition of care or transition to home to his/her Primary/Specialty Care
Provider .
10.
Based Primary Care if clinically
indicated at the conclusion of program enrollment.
11.
Collaborates with the Physician
Preceptor and Specialty Care Clinic(s) as needed to provide excellent health
care.
The position is responsible to the
Physician Preceptor for medical issues and administratively to the
Home Based Primary Care Pro! gram
Director.
Relocation

QUALIFICATIONS REQUIRED:
BASIC REQUIREMENTS:
- U.S. citizenship. Non-citizens may be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
- Full unrestricted Licensure, Certification, or Registration required
- Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia.
- Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The National League for Nursing Accrediting Commission (NLNAC) or The Commission on Collegiate Nursing Education (CCNE). In cases of graduates of foreign schools of professional nursing, posse! ssion of current, full, active and unrestricted registration will meet ! the requirement of graduation from an approved school of professional nursing.
EDUCATION/EXPERIENCE REQUIREMENTS:
Nurse II - A BSN with approximately 2-3 years of experience, or ADN or Diploma in Nursing and a bachelors degree in a related field and approximately 2-3 years experience or a Master's degree in nursing or related field with a BSN and approximately 1-2 years experience, or a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no experience.
Nurse III - Master's degree in! nursing or related field with BSN and approximately 2-3 years experience or Doctoral degree in nursing or related field and approximately 2-3 years experience.
Preferred Experience:
1.
MasterÂ's Degree in Nursing or related field with BSN or Doctoral degree in
nursing or related field .
2.
Certification as a clinical nurse specialist.
3.
A minimum of 3 years of cardiovascular nursing experience
4. ACLS and BLS prepared
