SUMMARY: Provides prescribed medical treatment and personal care services to ill, injured, convalescent, and disabled persons in such settings as the hospital, clinics, physician offices, private homes, schools, nursing homes, and similar institutions by performing the following duties.

SUPERVISORY RESPONSIBILITIES:
This job has no supervisory responsibilities.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

  • Provide exceptional service and/or care to all patients, customers and team members.
  • Interviews and assesses patient to determine medical problem/condition and documents in chart for physician.
  • Records all information accurately, concisely, and completely.
  • Obtains and records patient's vital signs and weight.
  • Assists physician with patient examination as needed.
  • Schedules tests or treatments at other facilities and forwards medical information as indicated to complete care.
  • Reviews physician's orders, lab requests, or follow-up needs with patient.
  • Documents services performed for billing purposes.
  • Observes patients and reports adverse reactions to medication or treatment to medical personnel in charge.
  • Administers specified medication, orally or by subcutaneous or inter-muscular injection, and notes time and amount on patients' charts as prescribed by provider.
  • Assembles and uses such equipment as catheters, tracheotomy tubes, and oxygen suppliers as prescribed by provider.
  • Calls and/or faxes prescriptions
  • Collects samples, such as urine, blood, and sputum, from patients for testing and performs routine laboratory tests on samples as prescribed by provider.
  • Participates in planning, implementation, and evaluation of nursing care plans and nursing care.
  • Participates in team conferences, sharing knowledge, information, and ideas among team members.
  • Performs more complex nursing activities as competence increases.
  • Participates in emergency care as needed.
  • Communicates frequently with the Team Lead regarding patients’ physical & mental conditions.
  • Prepares or examines food trays for prescribed diet and feeds patients.
  • Records food and fluid intake and output.
  • Bathes, dresses, and assists patients in walking and turning.
  • Assists in delivery, care, and feeding of infants.
  • Perform other duties as required.
QUALIFICATION REQUIREMENTS:
To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. Education and/or Experience:
  • High school diploma or general education degree (GED); and
  • One year certificate from college or technical school.
  • One year of clinical practice preferred
License(s), Certification(s), Registration(s), Accreditation(s)

  • LPN Licensure in the State of Missouri
  • BLS
  • I.V. Certification required for Acute nursing but not for Home Health & Hospice or Hyperbaric positions
  • Certification in specialty area preferred
Knowledge, Skills and Abilities:
  • Ability to communicate effectively.
  • Demonstrated clinical competence.
  • Ability to interpret a variety of instructions furnished in written, oral, or schedule form.
  • Ability to write routine reports.
  • Knowledge of and ability to use all clinical software
  • I.V. Insertion (does not apply to Home Health & Hospice positions)
  • Skilled in operation of tools, equipment, and materials listed below.
